Amusements Licenses

General & Mechanical License

An annual license expiring on January 31st.

This license applies to all mechanical devices used by the public for amusement and entertainment.  The devices may be operated by inserting a coin, slug, token, plate, disc, or card into the device.  Such devices include, but are not limited to, juke boxes, skill ball, video games, mechanical grab devices, pin ball machines, lottery games, and all coin-operated machines and games.  This license also applies to all billiard and pool tables used by the public for amusement and entertainment.  The tables may be coin operated or paid for based on time used.  See Code sections 5.219 through 5.224.

To obtain a license, complete the General and Mechanical Amusement License Application and return it with the appropriate fee to the Payment Center at City Hall.  The license will be issued upon the approval of the application
Compliance with all laws, ordinances, and regulations of the City of Rockford, including all building, zoning, and fire regulations as well as all State of Illinois statutes applicable to such a licensed business or device, is mandatory.

Amusement General and Mechanical

Bowling Alley License

An annual license expiring on January 31st.

This license applies to all bowling facilities used by the public for amusement and entertainment.  See Code sections 5.219 through 5.224.

To obtain a license, complete the Bowling Alley License Application and return it with the appropriate fee to the Payment Center at City Hall.  The license will be issued upon approval of the application.

Compliance with all laws, ordinances, and regulations of the City of Rockford, including all building, zoning, and fire regulations as well as all State of Illinois statutes applicable to such a licensed business or device is mandatory.

Bowling License Application

Movie Theater License

An annual license expiring on May 31st.

This license applies to all movie theaters used by the public for amusement and entertainment.  See Code sections 5.219 through 5.224.

To obtain a license, complete the Movie Theater License Application and return it wil the appropriate fee to the Payment Center at City Hall.   The license will be issued upon approval of the application.

Compliance with all laws, ordinances, and regulations of the City of Rockford, including all building, zoning, and fire regulations as well as State of Illinois statutes applicable to such a licensed business or device, is mandatory.

Movie Theater License Application

Skating Rink License

An annual license expiring on January 31st.

This license applies to all (ice, roller, and skate board) skating facilities used by the public for amusement and entertainment.  See Code sections 5.219 through 5.224.

To obtain a license, complete the Skating Rink License Application and return it wil the appropriate fee to the Payment Center at City Hall.  The license will be issued upon approval of application.

Compliance with all laws, ordinances, and regulations of the City of Rockford, including all building, zoning, and fire regulations as well as all State of Illinois statutes applicable to such a licensed business or device, is mandatory.

Skating License Application

Video Gaming Permit

Video gaming terminals may be allowed in an establishment in conjunction with a valid City of Rockford Liquor License that permits VGTs as a condition of the license. Any liquor license holder seeking to either add video gaming or increase the number of VGTs authorized in their establishment must apply for a modification to their existing liquor license.

• If you are an existing liquor license holder that is currently authorized to have video gaming terminals in your establishment, you must apply for a modification to your existing liquor license to add one additional video gaming terminal during any twelve (12) month period.

o Complete the application titled “Modification to Existing Liquor License- Add One Video Gaming Terminal” below.
o Submit the application and required documents as outlined in the Information-Instruction Sheet to the City of Rockford Department of Law.
o Applications to add one additional terminal are not required to go before the LTAB for consideration; they will go directly to the Code & Regulation Committee.
o Applications are due by 5:00 pm on the second Friday preceding the Code & Regulation Committee.
o There is no application fee to apply for a modification to an existing liquor license to add one additional terminal.

• If you are an existing liquor license holder without video gaming terminals you must apply for a modification to your liquor license for a new VGT-associated license in order to authorize any video gaming terminals in your establishment.

o Complete the applicable application titled “Application for a Beer Garden, Outdoor Liquor Sales, or Modification to an Existing Liquor License” below.
o Submit the application and required documents as outlined in the Required Attachments sheet to the City of Rockford Department of Law.
o Application to modify an existing liquor license for a VGT-associated license to add gaming at the establishment are required to go before the LTAB for consideration.
o Applications are due by the deadlines set out in the annual LTAB schedule.
o Applicants must pay the regular $870 fee to apply for a new VGT-associated license to authorize video gaming at the establishment.
